Request for Assistance from the ICS Insurance Relations Committee As a benefit of ICS Membership, we are pleased to offer you the assistance of our Insurance Relations Committee in addressing your insurance-related question or problem. In response to your request for the assistance of your colleagues on the ICS Insurance Relations Committee, they ask that you complete and return the attached Assistance Request Form along with any supporting documents. Sending the material by fax to 515/963-9361 will speed the process of getting a response, or place it in the mail. When completing the form, please print or type. If you require additional space, use additional pages and attach to the form. If you choose to attach other supporting documents, be certain to delete any information that would identify the patient(s). Once we have the Assistance Request Form in hand, we will either respond immediately using in-office documents compiled from similar requests from others or fax the request to the appropriate committee member who may contact you by phone for additional information or to respond to your request. This generally requires 1-3 business days. While we will respond as quickly as possible, these doctors are taking time away from their practice to respond to your request so please be patient. If your inquiry requires additional time for investigation, you will be notified. We hope to be of assistance to you in quickly resolving this matter or in referring you to the right person.
